Roster Forms & Instructions

1. Download and complete the correct Roster and Players’ Contract Form. Please type or print clearly. Click on your team’s classification to open the correct Roster/Players Contract Form (Opens a PDF Document):

2. Enter name of team and previous name and class if any.

3. Class: Circle one if applicable. The League Office reserves the right to reclassify any team. A team wishing to appeal its classification must submit the team roster along with a written letter indicating the reason for appeal. An appeals committee will hear the team’s appeal on June 15, from 6:30 p.m. to 9:00 p.m. A team will be given a time to meet with the appeals committee when the roster and appeal letter are submitted to the League Office.

4. Complete player information: Players’ residency will be determined by the address listed on their Driver’s License or State Identification Card. Players using Passports or having a Non-San Francisco address will be considered non-residents. Note: If a player does not live in San Francisco, he/she must work or go to school in San Francisco in order to play in this league.

5. Have players read and sign the Players Code of Conduct, Players’ Contract Form and Roster. All Rosters and Players’ Contracts must have a minimum of 15 players. Exception: The minimum for Co-Ed is eight men and eight women. Entries below the minimum requirement will not be accepted. The maximum number of players for Slow Pitch is 20 and for Fast Pitch, 18. Playing managers are counted within the total number of players permitted.

6. PLAYERS MAY PARTICIPATE FOR UP TO TWO (2) TEAMS, BUT NOT FOR TWO TEAMS IN THE SAME CLASSIFICATION. Example: A player may play for 1 Open team and 1 Co-Ed team, or 1 Seniors team and 1 Masters team, but may not play on 2 Open teams. See the Rules page for more detailed information.

7. List Manager’s and Captain’s name, address, and daytime and evening phone numbers. Please include proper area code.

8. IMPORTANT: “X” the days and nights your team cannot play.

Make sure your team can play on the days/nights you leave blank on the roster adjustment form. Once the schedule is established, your team is responsible to follow it. Any team dropping out of the league after the schedule has been printed will be charged a $150.00 administration fee.

You are allowed to make mid-season adjustments to your Team Roster. Please see the “Roster Adjustment” section below for more information.

Only ORIGINAL SIGNATURES ON ROSTERS AND PLAYERS’ CONTRACT FORMS will be accepted. Please make Xerox copies for your own use before submitting the Roster and Players’ Contract Form. Please bring your completed Roster Forms and Entry Fees to the League Office during the Team Registration period.

Entry Fees

Payment of fees must be made by money order or cashier’s check payable to SF Municipal Softball, VISA and MasterCard only.

Personal checks, business checks or cash will not be accepted. All rosters submitted after the deadline will be charged a $25.00 late fee.

All Co-Ed: $500.00*

All Slow Pitch (Industrial, Masters, Open, Women): $500.00*

All Fast Pitch: $515.00*

Note: Entry fee minus $150.00 will be refunded to teams that drop out before league play begins. No entry fee will be refunded once league play begins.

*An additional $5.00 fee is required for each non-San Francisco resident.

Residency will be determined as follows:

A player will be considered a San Francisco resident only if he/she has one of the following:
. California drivers license with a San Francisco address
. California identification card (DMVIC) with a San Francisco address

A player will be considered a nonresident if he/she has one of the following:
. California drivers license without a San Francisco address
. California identification card (DMVIC) without a San Francisco address
. Out-of-state license
. Out-of-state identification card
. Passport

Roster Adjustment Forms & Instructions

You are allowed to make mid-season adjustments to your Team Roster. To do so, you must submit a “Roster Adjustment Form” before the Roster Adjustment Deadline during the Season. Click on your team’s classification to open the correct Roster Adjustment Form (Opens a PDF Document):

1. Enter name of team and classification.

2. A team wishing to appeal its classification must submit the roster adjustment form along with a written letter indicating cause for appeal. An appeals committee will hear appeals. Each team will be given a date and time for appeal when roster adjustment form and letter are submitted.

3. Check and review completely the team roster for players who participated on your team.

Note: The minimum required for Co-Ed is 8 men and 8 women. The maximum number of players for Slow Pitch is 20 and for Fast Pitch, 18. Playing managers are counted within the total number of players permitted.

4. If there is a player’s name to be added to your team roster, list them on the roster adjustment form.

5. Have the added players on your team read the Code of Conduct and Players’ Contract form, then have them sign both the roster adjustment form and Player’s Contract form.

Note: Roster information and player’s contract form must match the player’s ID and signature.

Important: All players added to a roster and players who did not register at the start of the season must read and sign the Player’s Contract form.

If your last roster has a player with an improper signature, please use the roster adjustment form for correction. Remember, all protests involving player eligibility are determined by signature.

6. List manager’s and captain’s name, address, day and evening phone numbers only if changes are being made to those positions.

Note: This updated information is necessary for the league to ensure effective communication with teams.

7. Manager must sign the roster adjustment form and back of the Player’s’ Contract form.

8. Submit the Roster Adjustment Form to the League Office.
